Before completing an application for funding from Maddie's Fund, it is strongly recommended that you read the Introduction, Funding Guidelines and Instructions, FAQs, and Policy Matters found in this section.

The Preliminary Application (7 pages), the Adoption Application (15 pages) and the Spay/Neuter Application Instructions (4 pages) can be obtained by downloading and printing the Acrobat document versions or by sending an e-mail request to info@maddies.org.

Keep in mind that Maddie's Fund community grant applicants will be asked to submit two applications:
  • The adoption guarantee lead agency for the Adoption Component, entitled Maddie's® Pet Rescue Project, must submit the Maddie's Fund Adoption Application for a Community Collaborative Project.
  • The lead agency for the Spay/Neuter Component, entitled Maddie's® Spay/Neuter (S/N) Project, must submit the Maddie's Fund Spay/Neuter Application for a Community Collaborative Project.

We will not accept standalone Adoption or Spay/Neuter applications. These applications must be submitted concurrently, reflecting the joint commitment of the humane and veterinary communities to ending the killing of healthy shelter dogs and cats within five years.
